World Health Organization presents guidelines to make doctors do less during childbirth

Health professionals should trust women's ability to give birth naturally, WHO says in new guidelines that aim to reduce medical interventions in delivery rooms. Yet women are increasingly being subjected to medical interventions in the name of risk-avoidance, which may be unnecessary and unwelcome to them, says the WHO.

"However, what has been happening over the last two decades is we have been having more and more medical interventions being applied unnecessarily to women and we have situations where several woman are getting too many interventions that they do not need".

When doctors and other care providers confront labour moving slower than that rate, "the tendency is to act", either with a caesarean section or with the use of drugs like oxytocin that speed up labour, leading to the "increased medicalisation" of childbirth, he said. A woman's vital signs and baby's heartbeat should be monitored closely throughout.

"We want women to give birth in a safe environment with skilled birth attendants in well-equipped facilities".

Among 26 new recommendations, it rejected a traditional benchmark in labour wards worldwide for the dilation of a woman's cervix at the rate of 1 centimetre per hour, saying it was "unrealistic" and often led to excessive caesarean sections.

For a subsequent pregnancy, the figure drops to less than 10 hours.

To reduce unnecessary medical interventions, the World Health Organization guideline states that the previous benchmark for cervical dilation rate at 1 cm/hr during the active first stage of labour (as assessed by a partograph or chart used to document the course of a normal labour) may be unrealistic for some women and is inaccurate in identifying women at risk of adverse birth outcomes.

Health workers must respect women's wishes during childbirth, even if medical interventions are needed, the United Nations agency says in its new recommendations, which now include breathing techniques and music.

About 830 women die from complications in pregnancy or childbirth around the world every day. "The guidelines place the woman and her baby at the center of the care model, to achieve the best possible physical, emotional, and psychological outcomes" (2/15). While most women want a natural labour and birth, they also acknowledge that birth can be an unpredictable and risky event and that close monitoring and sometimes medical interventions may be necessary.
